Making a Referral
Referrals to Mayo Early Intervention Service are made by Public Health Nurses, GP’s, Paediatricians, Primary Care Services, Hospital Services, the relevant agencies.
Parental consent is required in order for a referral to be made to the service.
Families can refer directly to the service. Additional supportive information will be required to ensure the child meets the criteria for access to the service.
Criteria for Admission
Children who present with delays in two or more areas of motor, cognitive, communication and sensory function and who have complex needs requiring on-going team based interventions. Children outside of these criteria will be supported at Primary Care level.
Planning for Services and Supports to Child & Family
- Referrals Forum – Monthly
- Team named (based on presenting information/issues)
- Each child/family is allocated key worker (named at Referrals Forum – can change as circumstances arise)
- Initial visit to family carried out
- Information gathering process commences
- The service aims to foster a supportive partnership with family
- The service aims to identify the child & family’s needs, strengths and goals
- Family questionnaire s used as a guide for this purpose
- Feedback from initial visit is provided to other team members
- Follow up actions and planning process commences
- Review process in place and being developed further to review goals and plans in line with child/family’s changing needs and priorities
- Individual child and family service plans are developed to capture the child/family’s priorities and actions and supports and follow-up required
- The key worker will maintain contact with the family and keep the team informed of changing priorities
